A family member or caregiver can help you complete the survey.

The survey is available in:

Results of this survey will help the City plan and prepare for storms. Completing the survey does not mean that first responders will know your location in an emergency.

All responses will be kept strictly confidential. Information provided will only be used for analysis of the need of government transportation in the event of a coastal storm hitting New York City.

Storm Evacuation Zones

There are 6 hurricane evacuation zones. They are ranked by the risk of storm surge impact, with zone 1 being the most likely to flood. In the event of a hurricane or tropical storm, residents in these zones may be ordered to evacuate.
