Wheelchair Friendly Decal

Restaurants and other food service establishments that qualify, can obtain a Wheelchair Friendly decal that can be posted inside or at the entrance of their establishment. Applicants must complete a survey to determine if they qualify.

Applicants can complete the survey online by going to nyc.gov/dohmhpermits. Registration is required and you will need your permit Record ID or CAMIS number. Once logged in, you will go to Select an Online Service and choose Wheel Chair Decal from the list.

Applicants for new food service establishment permits will be able to apply for the decal when they apply for their permit.

Status

If you requested a Department of Health and Mental Hygiene publication over 2 weeks ago and have not received it, you can check the status. If you ordered it more recently, allow time for delivery and call back after 2 weeks if you still haven't received it.

Cancel Order

If your order has been placed, it likely already shipped. You can return unwanted copies at no cost to you by writing "Return to Sender" on the package and putting it in the mail.

To order additional items, you must place another order.
To update a current order, be sure to include tracking order number.
