Due to the recommendation from the Federal Drug Administration (FDA) and the Centers for Disease Control and Prevention (CDC), the City is pausing administration of the Johnson and Johnson vaccine as of Tuesday, April 13.

Out of an abundance of caution, if you have a vaccine appointment to receive the Johnson and Johnson vaccine at an NYC Health + Hospitals or Department of Health and Mental Hygiene (DOHMH) site, it may be rescheduled. If your appointment is rescheduled you will receive an email with the new details of your appointment. For other appointments that may be booked you will be rescheduled and contacted by your provider. You will receive the Moderna or Pfizer vaccine instead of the Johnson and Johnson vaccine until there is further guidance from Federal agencies.

The CDC is investigating a few cases related to the Johnson and Johnson vaccine and should have more information soon.

If you have received the Johnson & Johnson / Janssen vaccine in the past three weeks and are having any of the following symptoms, contact your health care provider:

  • Severe headache
  • Abdominal pain
  • Leg pain
  • Shortness of breath​

The COVID-19 vaccine can prevent COVID-19 symptoms and severe COVID-19 illness.

There are currently three COVID-19 vaccines authorized by the Food and Drug Administration:

  • Pfizer-BioNTech
  • Moderna
  • Johnson and Johnson

The vaccines are administered through shots in the arm. The Pfizer and Moderna vaccines require two doses taken 3 to 4 weeks apart. 

An appointment is required to get a vaccine. All New Yorkers 16 years of age and older are eligible to be vaccinated. People who work or study in New York are also eligible.

If you are ages 16 or 17, you may only receive the Pfizer vaccine at this time. Minors must have parental, guardian or legal custodian consent, unless the minor is part of a group to whom the law gives the right to consent to their own care (e.g., married minors, minors who are parents or pregnant, and minors in the military). A parent or legal guardian should accompany a minor to their vaccination or must be available by phone at the time of the minor’s vaccination to provide verbal consent to the provider.

The vaccine is free for all New Yorkers, regardless of immigration status. If you have insurance, it may be billed, but you will not be charged a copay or other fee.

Find a Vaccination Site

The Vaccine Finder is an online tool that can help you find the closest site to your location. You cannot search by which of the 3 vaccines is provided.

The Finder lists:

  • NYC Department of Health and Mental Hygiene (DOHMH) clinics and hubs
  • NYC Health + Hospitals sites
  • City-run mass vaccination sites (mega sites)
  • State-run sites, such as York College or Medgar Evers College 
  • Privately-run sites, such as urgent care centers and pharmacies *

* Pharmacies can only vaccinate people who are 30 years and older AND teachers, school staff, childcare workers, and people with eligible underlying conditions. All other sites can vaccinate any eligible person.

Each site listed in the Vaccine Finder includes a link or phone number to schedule an appointment. 

There is limited supply of vaccine from the federal government at this time. You may not see open appointments on the Vaccine Finder right now, but check back regularly. New appointments will open up once more supply becomes available. If you can’t find an appointment, please try again later. New locations and appointment slots are added every day.

If you don’t have internet access, you can schedule an appointment with the Vaccine Reservation Call Center. However, the reservation center can ONLY schedule an appointment for Health + Hospitals and NYC Department of Health vaccination sites. Translators are available.

What You Need to Bring

Before you can get a vaccine, you must fill out the New York State COVID-19 Vaccine Form. You can complete it online and bring your submission ID, or you can complete the form at your vaccination site.

You will get an email confirmation with a QR code if you have completed the screening questions. If you haven’t received the QR Code and your appointment is today, you should attend your appointment and finish processing at the facility.

You also need to bring proof of eligibility to your appointment.

Proof of Eligibility

You must bring proof that you live and work in New York City.

Proof of residency may include:

  • State or government-issued ID
  • Statement from your landlord
  • Current rent receipt or lease
  • Mortgage records
  • Both a statement from another person and current mail or school records

If you're eligible based on your employment, you must also bring proof of employment such as a:

  • Employee ID card
  • Letter from your employer or affiliated organization
  • Recent pay stub
  • Current, valid TLC License

If you're eligible for the vaccine based on your age, you must also show proof of age, such as:

  • Driver’s license or non-driver ID
  • IDNYC card
  • Birth certificate issued by a state or local government
  • Current U.S passport or valid foreign passport
  • Permanent resident card
  • Certificate of Naturalization or Citizenship
  • Life insurance policy with birthdate
  • Marriage certificate with birthdate

If you're eligible for the vaccine due to an underlying condition, you do not need documentation from your personal health care provider or any other proof of your condition to get a vaccine in NYC. It is only necessary for you to self-certify you have an underlying condition that makes you eligible. You will be asked to self-certify as part of the appointment scheduling process, or a vaccine provider will ask you to complete a certification document before or at the time of your appointment.

You may not be required to certify your condition if you are vaccinated by your usual health care provider. 

After the Appointment

After you get vaccinated at a City or State site, you'll be given a card with the following:

  • Your name, date of birth, and medical record number (if you have one)
  • Name of the vaccine you were given
  • Date and place you got the shot

If you get a Pfizer or Moderna vaccine, you should schedule a second-dose appointment before you leave.

The City is planning to open a COVID-19 vaccination mega site at the Barclays Center for eligible residents of Brooklyn. Specific dates will be available soon.

NYCHA vaccine sites are available to NYCHA residents who meet the eligibility requirements. You can get a vaccine from a NYCHA site only if you live in the development that it serves. Appointments are required.

Mobile Sites

The mobile vaccination effort includes a fleet of vaccine vehicles, including vans and buses, equipped with four to six vaccinators each, delivering the COVID-19 vaccine directly to communities most in need, whether priority populations or neighborhoods identified by the City’s Task Force for Racial Inclusion and Equity as being hardest-hit by the virus.

At these locations, the van will offer approximately 200 vaccine doses per day, for a range of one to three days. Local community-based organizations will help New Yorkers schedule vaccination appointments with the vaccine fleet ahead of time. The fleet will also accept walk-up appointments when available. 

Additionally, the City will assist with outreach to the community, helping to coordinate with local community-based organizations, elected officials, and industry groups to engage communities, post flyers, promote these opportunities, and fill these appointments. Vans will have live interpretation on site and vaccine materials in 12 languages.

Mobile Sites for Restaurant and Delivery Workers

The first bus will focus on reaching restaurant workers and delivery workers. Promobile Kitchen will operate this first bus, Daybreak Health will administer vaccine doses, and Relief Opportunities for All Restaurants (ROAR) will lead outreach efforts to restaurant workers, helping them sign up for appointments ahead of time and on site.

Details about additional mobile vaccine sites, including hours and days of operation, will be announced in the coming weeks.

New Yorkers aged 50 years or older can get vaccinated at the following locations at any time and without an appointment.

In addition, one individual escorting the 50+ New Yorker may also receive a vaccine without an appointment as long as they are a New York resident over the age of 16.

Before you can get a vaccine, you must fill out the New York State COVID-19 Vaccine Form. You can complete the form at your vaccination site. You can also complete the form online before going to your vaccination site.

If you choose to complete the form online, you will get an email confirmation with a QR code. If you haven’t received the QR Code and you plan to get your vaccine today, you should still go to the vaccination site and finish processing at the facility.

You can schedule transportation to your vaccine appointment if you are 65 or older and:

  • Cannot use public transportation,
  • Cannot use private transportation, and
  • Cannot rely on friends or family members for transport.

You must already have a vaccine appointment before scheduling transportation. However, you do not need an appointment for sites listed in the "Walk-Up Appointments for Seniors 75 or Older" section above, if you are a senior aged 75 or older.

If you already use Access-a-Ride, you can schedule transportation to your appointment as you normally would.

Medicaid

If you have Medicaid-provided transportation, you can use your usual contact number to schedule transport. You can also call Medicaid Transportation Management.

Curb

If you are 65 or older and meet all of the criteria, but do not qualify for the programs above, you can schedule a free ride with Curb.

When scheduling your appointment, you must indicate that you are getting a vaccine. Curb will confirm that your destination is a valid vaccination site, and your ride will be scheduled for free.

The Pfizer-BioNTech and Moderna COVID-19 vaccines require two doses that must be taken 3 to 4 weeks apart.

At your appointment for the first dose, you should schedule a second-dose appointment. If you left without scheduling the second appointment, you should contact the site where you got your first shot.

If you got your first dose from an NYC Health + Hospitals or NYC Department of Health and Mental Hygiene (DOHMH) site, you can confirm your scheduled appointment or make a second appointment with the Vaccine Reservation Call Center.

If you got your first dose from DOHMH, you can also schedule a second-dose appointment online.

Vaccine appointments at City-run sites may be rescheduled due to vaccine shortages or severe weather. If you are impacted, the City will call and email you with a new date and time to show up for your appointment.

If you need to reschedule or cancel a vaccine appointment, contact the site where you scheduled your appointment.

If you have an appointment with an NYC Health + Hospitals or NYC Department of Health and Mental Hygiene (DOHMH) site, you can cancel or reschedule your appointment with the Vaccine Reservation Call Center.

Homebound vaccinations have resumed with limited capacity using the Moderna vaccine.

In-home COVID-19 vaccinations are available for eligible NYC residents who are:

  • Fully homebound, 
  • Have not already been vaccinated, and 
  • Do not already have access to a vaccination program.

If you are eligible for an in-home vaccination, you can express interest using the form below. The City will use this information to call back to further discuss eligibility for the program.

The Vaccine for All Corps is hiring New Yorkers to work at vaccine sites.

The City is hiring directly from the community and prioritizing applicants from neighborhoods most impacted by COVID-19.

No healthcare experience is required for non-clinical roles, which include management, operations, and client services.

Clinical roles, including vaccinators, are also available.

New York State’s Excelsior Pass provides secure, digital proof of COVID-19 vaccination or negative test results. The Pass is only accepted in New York State.

Businesses and entertainment venues can scan and validate the pass to ensure you meet any COVID-19 vaccination or testing requirements for entry. When using the pass, you’ll be asked to show a photo ID that shows your name and birth date to verify identity.

The Excelsior Pass can digitally store information on your smartphone with the Excelsior Pass Wallet app, available for free from the Apple App Store and Google Play Store. You can also print your Pass from the Excelsior Pass website and bring it with you.

Participation in Excelsior Pass is voluntary. You can always show alternate proof of vaccination or testing, like another mobile application or paper form, directly at a business or venue.

Once you enter an establishment, you must continue to follow social distancing, face covering and hand hygiene guidelines.

Using Excelsior Pass

The COVID-19 Vaccination Pass is currently valid for 30 days after the Pass is retrieved. After the pass expires, you can retrieve a new Pass from the Excelsior Pass website.

The COVID-19 PCR Test Pass is valid until midnight on the third day after your test. After your COVID-19 PCR Test Pass expires, a new test is required to retrieve a new Pass.

The COVID-19 Antigen Test Pass is valid for 6 hours after your test. After your COVID-19 Antigen Test Pass expires, a new test is required to retrieve a new Pass.
